Using an Application Form is a great way to gather more information from potentail participants. You may use the information provided to vet particiaption or, if you’re forming more than one group, group participants together. Below are some sample questions you might ask during the application process. You don’t have to use all of thes questions, and may add more depending on your needs. You’ll sumbmit your preferred questions via the “Application” tab on the Growth Group Master document.
The following questions are required for every application.
What is your first name?
Last name?
Preferred email address?
Mailing address (we’ll only use this info to send you a welcome note, and never sell it to anyone)?
Name of the company or organization that is organizing your group?
Do you acknowledge that groups meet once a month and 100% participation is expected?
Do you acknowledge that your group will meet on [scheduled meeting date/time]?
